Recently, I had a Type 1 ladder fail and drop me to the ground.  I am trying to figure out what happened since I , and familly have used this model for years.

I  deleted the name of manufacturer from text since I DO NOT have an issue with their quality.  I am just trying to determine failure mode to enhance my future safety.
